These are useful preparation items, not required uploads. Quick and full requests can start with your name and one contact method. No EIN is requested.
Your declarations page or coverage summary, renewal date, and any cancellation or non-renewal notice. New business? Say this is your first policy.
Year, make, model and VIN for each unit; garaging location and insured value where relevant. Include units you plan to add or remove. A CSV schedule saves typing.
Your current driver list, license number and state, and driving experience. Add hiring information if requested. Only share driver information you are authorized to provide.
Available loss-run reports from your current and previous insurers. Ask the reviewing agent which years and how recent the reports need to be.
What you haul, operating states and radius, mileage, fleet changes, and when you need coverage. A voice note can explain details that do not fit a form.
FOR LARGER OR MORE COMPLEX FLEETS
A reviewing market may also ask for mileage or IFTA/ELD reports, safety and driver-training materials, MVRs, or financial statements. Upload these only when relevant and requested. Requirements depend on the insurer and operation.
Loss runs are insurer-issued reports of your claims history. Request current reports from your current and previous insurers or agents. The reviewing agent should confirm the required years and report freshness; some fleet programs ask for five years.
